Licensed Independent Clinical Social Worker for Milford Regional Physician Group

Statement of Purpose
The Licensed Clinical Social Worker evaluates a wide range of psycho-social problems applying individual clinical social work skills in order to assist patients and their families/significant others in the resolution of personal and environmental problems, attributable to mental or physical illness, which prevent them from achieving their highest level of psycho-social functioning.  The Licensed Clinical Social Worker will work throughout the group practices of Milford Regional Physician Group and will provide support at patients' homes as well as Milford Regional Medical Center.

Education:   MSW, Master Social Work
Certification/License:  LICSW - Preferred or LCSW- Expectation is that employee will obtain LICSW within 2 years of employment.   
Experience:  Two years of experience in evaluating the clinical needs and system of care needs for patients. Analytical and Case Management skills necessary to evaluate patients, develop treatment plans, initiate therapy, and evaluate results.
Preferred Skills:  Ability to establish good rapport with patients, families, medical staff and outside agencies in providing services, reaching consensus and problem resolution.  Comfort in working in a team approach.
